Association between the Integrity of the External Limiting Membrane and Ellipsoid Zone and Visual Outcome in Retinal Vein Occlusion-Macular Edema

Abstract:Objective: To investigate the correlation between the integrity of the external limiting membrane (ELM) and ellipsoid zone (EZ) and the prognosis visual acuity of retinal vein occlusion-macular edema (RVO-ME) patients with the help of spectral-domain optical coherence tomography (SD-OCT) image features of retinal vein occlusion macular edema (RVO-ME). Methods: In this case series study, 30 consecutive patients (30 eyes) with initial RVO-ME diagnosis from Shanxi Eye Hospital between January 2019 and January 2022 were randomly selected according to the nanorow criteria and were examined by best corrected visual acuity (BCVA), intraocular pressure, slit lamp, SD-OCT, and CMT, ELM, EZ defect lengths within 500 μm through the fovea of the macula were measured with a caliper from an OCT self-contained device. Spearman correlation was used to analyse the correlation between the length of the ELM and EZ defects and CMT values at initial diagnosis with visual acuity improvement after treatment; The correlation of initial BCVA (LogMAR) with post-treatment BCVA was compared; We also analyzed the correlation between post-treatment CMT, and ELM and EZ defect lengths and post-treatment BCVA. Results: Visual acuity improvement values after treatment were negatively correlated with initial CMT (r=-0.67, P<0.001), initial ELM defect length (r=-0.82, P<0.001), and initial EZ defect length (r=-0.79, P<0.001), but not with age of onset (r=-0.18, P=0.540). There was a positive correlation between initial BCVA and post-treatment BCVA(r=0.67, P<0.001). Post treatment BCVAwas positively correlated with post-treatment CMT (r=0.38, P=0.040), ELM defect length (r=0.54, P=0.002), and EZ defect length (r=0.39, P=0.030). Conclusions:ELM and EZ integrity can be used as important indicators for determining the prognosis visual acuity of RVO-ME patients, and the initial visual acuity is also an important factor for determining the prognosis visual acuity of RVO-ME patients after edema resolution.
