有晶状体眼后房型人工晶状体植入术矫正中高度近视的远期临床观察

Abstract Objective: To observe the long-term efficacy of the implantable collamer lens (ICL) for the correction of moderate and high myopia. Methods: This was a retrospective study. Patients from 3 sites (Wuhan, Chongqing and Changsha Aier Eye Hospitals) who completed 5 years or more of follow-up were selected. Patients had undergone ICL surgery from December 2009 to December 2014. Visual acuity, sphericalequivalent (SE), intraocular pressure, corneal endothelial count (ECD), axial length (AL) and a visual quality questionnaire were recorded before surgery and after surgery. A paired t test and Pearson correlation were used for statistical analysis. Results: A total of 320 eyes from 168 patients were selected. The average follow-up time was 5-9(5.9±1.0) years postoperatively. Preoperative SE and SE at the last follow-up were -4.38- -18.25(-10.47±3.08)D and 0.38- -4.00(-0.91±0.82)D, respectively. At the last visit, the safety index and the effective index were 0.15-1.71(1.10±0.20) and 0.33-2.00(0.86±0.30), respectively. The UCVA and BCVA at the last visit had both improved compared to preoperative UCVA and BCVA (t=58.903, 6.637, P<0.001). The postoperative AL slightly increased (t=-9.136, P<0.001) and ECD decreased by 5.7% (t=10.077, P<0.001) and the differences were statistically significant. The ICL vault was 24-1 300(402±185)um at the last visit. The visual quality questionnaire showed that 95% of patients were satisfied with the surgery. Night vision and night driving had the lowest scores. Thirty-two eyes (10.0%) had complications or additional surgery including cataract surgery (5 eyes, 1.7%), ICL repositioning (10 eyes, 3.1%), ICL exchange (9 eyes, 2.8%), ICL explantation (2 eyes, 0.6%), traumatic retinal detachment (3 eyes, 0.9%), vitreous hemorrhage(1 eye, 0.3%), choroidal neovascularization (1 eye, 0.3%) and macular hemorrhage (1 eye, 0.3%). Conclusions: During more than 5 years of follow-up, the ICL demonstrates high safety, efficacy and satisfaction in moderate to high myopic correction, with a very low rate of complications.
