513 Safety and credibility of myopic corneal refractive surgery
LI Ying

Two decades of clinical retrospective studies have proven that myopic corneal refractive surgery is safe, effective and predictable.Ninety-eight percent of patients could obtain good refractive correction after the surgery with a long-term complication rate lower than 2%.And 96% of patients undergoing the surgery were content with postoperative outcomes, which were closely related to painlessness during the surgery, quick recovery and good uncorrected visual acuity after the surgery, advanced technology, various kinds of equipment, and an individualized operation plan. However,no more than 50% of the people were confident about the long-term safety of the surgery.Postoperative complications, refractive changes in high myopic patients, refractive regression, residual corneal turbidity and poor postoperative care should all account for this situation.Crucial factors that could enhance patient satisfaction and improve the degree of trust in myopic surgery include improvement of surgical safety, protection of the high quality of surgery, paying close attention to long-term changes in corneal and ocular status, and enhancement of the quality of care.Today in China,we should focus our problem-solving efforts on strict training of the surgical staff on standard operations, timely updates of surgical instruments and equipment, special care for the ocular changes in patients whose myopia is extremely high, and establishment of a policy to report serious surgical complications.

517 Clinical efficacy of wavefront-guided LASIK surgery with iris recognition
WU Jun-shu,GAO Shao-hui,ZHONG Xing-wu,ZHANG Xiao-xiao,YANG Bin,WANG Zheng,YU Ke-ming

Objective To study the clinical effects of myopia and myopic astigmatism treated by wavefront-guided laser in situ keratomileusis (LASIK) surgery with iris recognition. Methods In a prospective case-control study,LASIK surgery was performed on 30 eyes of 30 patients. One of the inclusion criteria was the root mean square value of higher order aberrations (RMS HO) no less than 0.30 μm preoperatively. The patients were randomly divided into group A and group B.Wavefront-guided LASIK surgery with iris recognition was used on the patients in group A,while conventional LASIK surgery was used on the patients in group B.Wavefront aberrations and contrast sensitivity functions (CSF) were measured and recorded preoperatively and at 1 month and 3 months postoperatively.Comparisons of the variables between groups A and B were done with an independent samples t test. Results Three months postoperatively, the mean log MAR visual acuities of groups A and B were -0.12±0.06 and-0.09±0.07,respectively.The differences were not statistically significant. One month postoperatively the mean RMS HO of group A was significantly lower than that of group B (t=-0.297,P<0.05).Three months postoperatively, photopic CSF without glare at 1.5 c/d and 3.0 c/d and mesopic CSF without glare at 18 c/d spatial frequency in group A were obviously better than in group B.The differences were statistically significant (t=2.517,2.175,2.263,P<0.05).Conclusion Wavefront-guided LASIK surgery with iris recognition is safe and effective in the treatment of myopia and myopic astigmatism.Wavefront-guided LASIK surgery with iris recognition has obvious superiority over conventional LASIK surgery in the correction of higher-order aberrations, which could ameliorate postoperative visual function deterioration after LASIK.

HU Qi,WU Qiong,WANG Ke-meng,KANG Yang
Objective To compare changes in wavefront aberrations on the anterior and posterior corneal surfaces after laser in situ keratomileusis (LASIK) and laser-assisted subepithelial keratectomy (LASEK) for myopia, especially for spherical aberrations, coma, trefoil and total higher order aberrations. Methods A prospective study was designed for 68 eyes of 34 patients who had undergone LASIK and LASEK.The patients were divided into 2 groups according to the procedure used.An oculyzer analytical system was used to measure corneal aberrations before and after surgery.Data were analyzed using ANOVA,independent samples t test and pearson correlation. Results One and 3 months after surgery, spherical aberrations and total higher-order aberrations of the anterior corneal surface in the LASIK group increased (P<0.01),but the differences at 1 month and 3 months were not significant (P>0.05).The same results occurred in the LASEK group for spherical aberrations, coma and total higher-order aberrations of the anterior corneal surface.Spherical aberrations and total higher-order aberrations of the anterior corneal surface were higher in the LASIK group than in the LASEK group 1 month after surgery (P<0.01),and the same tendency was found 3 months after surgery. However, coma aberrations on the anterior corneal surface in the LASIK group were less than those in the LASEK group at any time after surgery (P<0.05).No differences were found in trefoil aberrations on the anterior corneal surface during both procedures (P>0.05).Furthermore,the amount of correction achieved was positively correlated to these changes.Wavefront aberrations on the posterior corneal surface remained constant with both procedures and there were no differences when the procedures were compared (P>0.05).Conclusion Spherical aberrations, coma and total higher-order aberrations on the anterior corneal surface increased after both procedures, and did not recover until the 3 month after surgery. The changes in spherical aberrations and total higher-order aberrations on the anterior corneal surface were higher in the LASIK group than in the LASEK group, and depended on correction achieved in both procedures. The two procedures had no effect on wavefront aberrations on the posterior corneal surface.

QIAN Zhi-gang,KE Min,HUANG Gang,ZOU Juan
Objective To assess the clinical effectiveness of photorefractive keratectomy (PRK)versus laser subepithelial keratectomy (LASEK) for the correction of myopia. Methods A meta-analysis of the literature was conducted that included a review of randomly controlled trials in Pubmed,EMbase,National Knowledge Infrastructure and CQVIP.Data were selected by two independent reviewers using an extraction method. The results included efficacy outcomes (UCVA≥0.8,UCVA≥ 1.0postoperatively),accuracy outcomes (proportion of eyes within ±0.50 diopters of the target refraction postoperatively),safety outcomes (loss of ≥2 lines of BSCVA postoperatively),mean pain scores on day 1 postoperatively, timeline for complete healing of epithelial defects and mean corneal haze scores postoperatively. The Cochrane Collaboration's RevMan 5.0 software was used for statistical analysis. Results Nine RCTs involving 884 eyes with myopia were included.Meta-analysis showed that the efficacy (OR=1.08,95%CI:0.62-1.89),accuracy (OR=0.81,95%CI:0.53-1.24) and safety outcomes between PRK and LASEK were comparable.Compared to PRK,LASEK was better in reducing postoperative corneal haze intensity (WMD=0.23,95%CI:0.13-0.33).Conclusion PRK and LASEK have the same effectiveness, accuracy and safety for myopia, but LASEK results in less corneal haze intensity than PRK postoperatively.

551 Posterior chamber phakic intraocular lens implants for high myopia
LUO Yan,HU Liu-mei,XU Jing,LI Ying
Objective To evaluate the efficacy, safety, and biocompatibility of a posterior chamber phakic intraocular lens (PCPIOL) to correct high myopia within 1 year. Methods In a retrospective case series study,an implantable intraocular lens was implanted in 290 eyes of 149 patients with a mean age of 29.0±6.5 years (range 20 to 49 years). There were 107 eyes from males and 183 eyes from females.Mean preoperative myopia was-13.2±5.6 D (range-6.0 to-32.0 D).The follow-up included tests for visual acuity, intraocular pressure, manifest refraction, and adverse reactions. Results Postoperatively, all eyes had a significant increase in uncorrected visual acuity (UCVA).Best corrected visual acuity (BCVA) had improved by 1 or more lines in 199 eyes (68.6%)at 1 year.No eye lost BCVA.Thirty-two eyes developed intraocular pressure (IOP) more than 35 mmHg within 3 hours after the implatation.Pupillary blockage caused an increase in IOP in 13 eyes and insufficient removal of viscoelastic material caused an increase in 19 eyes.IOP returned to normal in the following days after quick treatment.Three eyes had intraocular pressure between 21-25 mmHg.Two eyes had macular hemorrhage 3 months after the implantation, and the hemorrhage regressed 3 month later.Throe IOLs were removed from 2 patients because of an intolerant halo. The IOLs were exchanged in 12 eyes in 1 year because of high or low vault. Conclusion After 1 year of follow-up,IOL implantation had good visual and refractive results with excellent biocompatibility and fewer adverse reactions.

574 Current status of keratectasia after laser in situ keratomileusis
XU Jing,LI Ying
Laser in situ keratomileusis (LASIK) is an established corneal refractive surgery.Keratectasia after LASIK is a dormant and serious complication that is still one of the most perplexing and widely discussed clinical issues facing refractive surgeons. An understanding of keratectasia after LASIK,including etiology, pathology, risk factors, preventions and treatments,is of great practical significance. This article reviews the current status of these aspects.
