Two decades of clinical retrospective studies have proven that myopic corneal refractive surgery is safe,effective and predictable.Ninety-eight percent of patients could obtain good refractive correction after the surgery with a long-term complication rate lower than 2%.And 96% of patients undergoing the surgery were content with postoperative outcomes,which were closely related to painlessness during the surgery,quick recovery and good uncorrected visual acuity after the surgery,advanced technology,various kinds of equipment,and an individualized operation plan.However,no more than 50% of the people were confident about the long-term safety of the surgery.Postoperative complications,refractive changes in high myopic patients,refractive regression,residual corneal turbidity and poor postoperative care should all account for this situation.Crucial factors that could enhance patient satisfaction and improve the degree of trust in myopic surgery include improvement of surgical safety,protection of the high quality of surgery,paying close attention to long-term changes in corneal and ocular status,and enhancement of the quality of care.Today in China,we should focus our problem-solving efforts on strict training of the surgical staff on standard operations,timely updates of surgical instruments and equipment,special care for the ocular changes in patients whose myopia is extremely high,and establishment of a policy to report serious surgical complications.
Objective To study the clinical effects of myopia and myopic astigmatism treated by wavefront-guided laser in situ keratomileusis (LASIK) surgery with iris recognition.Methods In a prospective case-control study,LASIK surgery was performed on 30 eyes of 30 patients.One of the inclusion criteria was the root mean square value of higher order aberrations (RMS HO) no less than 0.30 μm preoperatively.The patients were randomly divided into group A and group B.Wavefront-guided LASIK surgery with iris recognition was used on the patients in group A,while conventional LASIK surgery was used on the patients in group B.Wavefront aberrations and contrast sensitivity functions (CSF) were measured and recorded preoperatively and at 1 month and 3 months postoperatively.Comparisons of the variables between groups A and B were done with an independent samples t test.Results Three months postoperatively,the mean log MAR visual acuities of groups A and B were -0.12±0.06 and-0.09±0.07,respectively.The differences were not statistically significant.One month postoperatively the mean RMS HO of group A was significantly lower than that of group B (t=-0.297,P<0.05).Three months postoperatively,photopic CSF without glare at 1.5 c/d and 3.0 c/d and mesopic CSF without glare at 18 c/d spatial frequency in group A were obviously better than in group B.The differences were statistically significant (t=2.517,2.175,2.263,P<0.05).Conclusion Wavefront-guided LASIK surgery with iris recognition is safe and effective in the treatment of myopia and myopic astigmatism.Wavefront-guided LASIK surgery with iris recognition has obvious superiority over conventional LASIK surgery in the correction of higher-order aberrations,which could ameliorate postoperative visual function deterioration after LASIK.
Objective To investigate the clinic curative effects and rotational stability of AcrySof Toric intraocular lens (IOL) for corneal astigmatism eorrection in cataract patients.Methods It was a self control study.Different types of AcrySof Toric IOL were implanted in 25 cases (30 eyes)cataract patients with more than 1 D corneal astigmatism by phacoemulsification.Preoperation IOL spherical degree was calculated by ophthalmologic A/B scan.Corneal astigmatism was measured by corneal topography.Lens type and position were calculated by Alcon formula.During the operation,the axis of the Toric lens was alingned to the steepest axis of the corneal astigmatism.Patients were followed up for 3 months.The uncorrected visual acuity,corneal astigmatism,position of the lens and cylinder power were detected and compared before and after the operation.Data were analyzed using paired t test.Results One day postoperation,the mean uncorrected visual acuity (UCVA) was (4.72±0.12).At 3 months,it was (4.96±0.08).Preoperation the mean corneal astigmatism was (1.80±0.45)D,anticipated residual astigmatism was (0.29±0.16)D,3 months postoperation residual astigmatism was (0.33±0.21)D.The difference between preoperative corneal astigmatism and postoperative residual astigmatism was significant (t=3.39,P<0.05).And the difference between anticipated and actual residual astigmatism was not significant.The mean axis rotation of toric IOL was (5.50±4.07)° 1 day postoperation and (5.68±4.12)° after 3 months.IOL in 14 eyes had rotated less than 2°,IOL in 12 eyes had rotated 2-5°,and IOL in 4 eyes had rotated 5-10°.Conclusion Implantation of Acrysof Toric IOL for correction corneal astigmatism in cataract patients is effective and stable.
